You can also visit a DHSS office in person. You can find the nearest office by visiting the DHSS website or call the DHSS office in your area and ask for the location of the nearest food stamp office.
You may be asked to provide certain documentation, such as proof of identity and income, in order to request a new card, and it may take several days for a new card to be issued and sent to you.
