There are three ways to apply for AZ EBT benefits:

Online: You can apply online through Health-e-Arizona Plus. To do this, you will need to create an account and complete the online application.

By phone: You can call 1-855-432-7587 to apply for benefits over the phone.
In person: You can apply in person at any Arizona Department of Economic Security (DES) office.
To apply for AZ EBT benefits, you will need to provide the following information:

Your name, address, and date of birth
Your Social Security number or Individual Taxpayer Identification Number (ITIN)
The income and expenses of your household
The names and ages of all household members
Proof of identification for all household members
Proof of income for all household members
Proof of expenses, such as rent or mortgage statements, utility bills, and medical bills
If you are approved for AZ EBT benefits, you will be issued an EBT card. The EBT card is a debit card that can be used to purchase food at authorized retailers.
